Hallo!
Please send us your NVIDIA System Information report, full name of your graphics card and PSU (model name and manufacturer).
Here is how to create NVIDIA System Information report:
1. Launch NVIDIA Control Panel
2. Click System Information
3. In newly opened window click "Save"
I would also appreciate if you could send us .crashinfo file which is generated after the crash and can be found here: (...)\GOG Games\The Witcher 3\bin\x64
Thank you for sending NVIDIA System Information and .crashinfo file.
Try to use Display Driver Uninstaller
Display Driver Uninstaller Download version 15.5.1.0 to uninstall currently installed NVIDIA Driver.
After the uninstallation is complete, install NVIDIA Driver 353.30:
NVIDIA DRIVERS GeForce Game Ready Driver WHQL
The advice that I'm going to provide in a moment is hardware-related.
CD PROJEKT S.A. can only be responsible for problems that originated in the game itself and we are not obliged to provide support in matters related to hardware malfunction. However, we would like all the fans of our games and services to be able to enjoy them as much as possible and that is why we do our best to provide advice even in matters not directly related to the game.
At the same time, as we are not hardware manufacturers our suggestions should be treated as non-binding recommendations - we are not able to assess the source of the problem with full certainty without direct access to a given device, and we cannot be held accountable for direct or indirect damage resulting from following our advice. That is why if you are unsure whether you want to follow our non-binding suggestions, we recommend contacting the manufacturer of hardware in question.
We can recommend that you:
1. Download the nVidia Inspector here -
NVIDIA Inspector Download version 1.9.7.3.
2. Extract and run nvidiaInspector.exe. You will need Microsoft NET Framework as well.
3. Click the show overclocking button -

4. Hit the -20 button under Base Clock Offset three times to downclock your GPU by -60 MHz. Click Apply Clocks & Voltage.
5. Play The Witcher 3.
